The Client understands that the regular cost of the program is Health Coaching Sessions: 125.00 per hour.

Health Coaching Package:

#1 Three 1-hour sessions: 350.00

#2 Four 1-hour sessions: 450.00

#3 Five 1-hour sessions: 525.00

#4 Ten 1-hour sessions: 1000.00

#5 Twelve 1-hour sessions: 1125.00

The book titled “Preventative Health Pathway” written by M. Jill Keenon costs $30.00 plus shipping and handling of $6.00.
